This summary describes how Lumen Health PLLC may use and disclose your health information and your rights regarding that information. Our duties
We are required by law to maintain the privacy of your protected health information (PHI), to give you this Notice, and to follow the terms of this Notice. We will let you know if we change our privacy practices and provide an updated Notice.
How we may use and disclose your information
Treatment: To provide telehealth consultation services and coordinate referrals or follow-up at your request.
Payment: To obtain payment for services you receive (direct-pay processing and related activities).
Health care operations: For quality improvement, licensing, training, auditing, and administrative purposes.
Individuals involved in your care / your designee: With your permission or as allowed by law.
Required by law / public health & safety: For reporting certain conditions, preventing serious threats, responding to court orders and subpoenas, law enforcement requests, and other legally required disclosures.
Business associates: We may share PHI with vendors who perform services for us (e.g., telehealth platforms, EHR, billing) under written agreements requiring them to protect your information.
Other uses and disclosures: We will not use or disclose your PHI for marketing, sell your PHI, or use it for most research without your written authorization. Uses and disclosures not described in this Notice will be made only with your authorization, which you may revoke in writing at any time.
Your rights
Right to access: You may inspect or obtain a copy of your records (in paper or electronic format) within the timeframes allowed by law.
Right to amend: You may request corrections to your records if you believe information is inaccurate or incomplete.
Right to confidential communications: You may request that we contact you in a specific way (e.g., at a different address or email).
Right to request restrictions: You may ask us to limit certain uses/disclosures; we will consider your request but may not be able to agree in all cases.
Right to an accounting of disclosures: You may request a list of certain disclosures we have made of your PHI.

Right to be notified of a breach: You will be notified if a breach of unsecured PHI occurs that affects you.
Our responsibilities and safeguards
We use reasonable administrative, technical, and physical safeguards to protect your information, including secure telehealth platforms and secure messaging for sensitive information. Email may not be fully secure; if you prefer not to use email for PHI, please tell us and we will accommodate reasonable requests.
